GRAND JUNCTION, Colo. (AP) — Colorado officials have been unable to determine exactly what caused a fatal fall at a Grand Junction indoor recreation park in January.

The Daily Sentinel reported Tuesday that 20-year-old Ciara Romero fell to her death after she jumped from a nearly 80-foot-tall (24-meter-tall) platform at Get Air at the Silo.

Mahesh Albuquerque, director of the state's Division of Oil and Public Safety, which oversees amusement attractions, said the accident was unprecedented for the state.

Albuquerque says the division's findings did not include a reason as to why the carabiner-and-harness system employed by the ride failed.

There are six rides in Colorado that utilize the QUICKJump XL free fall device, and all six have been closed in the wake of the investigation.
